Download the tracks in .wav -format from the Internet Archive and burn them on a regular CD-R.

Open the pdf and print it. Remember to set the page scaling to none.

The printed version looks like this. Now some folding!

 

Fold the sides according to the marks. A ruler might be handy.

 

When the sides are done, fold the ends at their marks too.

 

Excellent!

 

Finally, fold the corners. It makes it easier to seal the covers.

 

Place the CD under the bottom end of the cover. It should fit like a glove.

 

Turn the upper end - the one with the logo - under the bottom.

 

Finished!
